Dans la médecine occidentale, le patient fut longtemps conçu comme un enfant à sauver, incapable ne serait-ce que de recevoir, de manière raisonnable, le savoir du médecin. Après la seconde Guerre Mondiale, la découverte des expérimentations scientifiques intolérables menées sur l’homme, mais aussi le vent libéral et individualiste des années 60, le progrès des biotechnologies, la démocratisation des savoirs ont favorisé l’émergence d’un nouveau paradigme et d’une nouvelle discipline : la bioéthique. Aujourd’hui, le médecin ne peut plus imposer son traitement (...) au malade, il doit, par respect de son autonomie, chercher à obtenir son consentement libre et éclairé. Les normes médicales et sociales, autrefois coercitives, voire transcendantes, sont aujourd’hui discutées et interrogées au regard des préférences et des valeurs individuelles. For centuries, heirloom rice varieties have been grown on the terraces of the Cordillera Mountains of Luzon, Philippines, terroirs known for their significant historical, cultural, and aesthetic values. However, heritage heirloom rice farming is gradually being abandoned, mainly because of its lower productivity and the struggle of the sector to create a sustainable niche market for heirloom rice by branding its cultural, social, and nutritional values. We propose several demand-side intervention strategies for the valorization of heirloom rice. To support the (...) development of a segmented marketing strategy for heritage farming, we provide evidence on urban consumers’ willingness to purchase heirloom rice. We interviewed 500 urban consumers from Metro Manila in July–August 2015, who placed a purchasing bid on a kilogram of heirloom rice. Consumers’ bids averaged PHP 72.61 kg−1, which is less than half its current market price. This explains why heirloom rice struggles to gain market share in urban markets in the Philippines. Given this bid price, we estimate a potential market size of PHP 20.3 billion that could be created for heirloom rice and tapped into by heritage farmers. Findings further indicate that women, business owners, and consumers who buy packaged rice and eat pigmented rice are willing to pay more for heirloom rice. Finally, our evidence suggests that proper information framing will be necessary to create demand and support the valorization of heirloom rice to preserve cultural heritage and in situ biodiversity of rice landraces in the Philippines. Faced with the threat of pandemic influenza, several countries have made the decision to put a number of measures in place which have been incorporated into national plans. In view of the magnitude of the powers and responsibilities that States assume in the event of a pandemic, a review of the various national preparedness and response plans for pandemic influenza brought to light a series of extremely important ethical concerns. Nevertheless, in spite of the recent emergence of literature focusing specifically (...) on the ethical aspects of pandemics, too few studies explicitly examine public participation as one means of ethical contribution to public health policies. Thus this article seeks, in the first place, to present an analysis of the various national preparedness and response plans for pandemic influenza, and secondly, to outline the role that the plans envisage for ethics and more importantly for public participation. In their book Spare Parts, published in 1992, Fox and Swazey criticized various aspects of organ transplantation, including the routinization of the procedure, ignorance regarding its inherent uncertainties, and the ethos of transplant professionals. Using this work as a frame of reference, we analyzed articles on organ transplantation published in internal medicine and transplantation journals between 1995 and 2008 to see whether Fox and Swazey’s critiques of organ transplantation were still relevant.

Résumé Bien qu’il soit très peu pris en compte par l’histoire des ordinateurs, l’analyseur différentiel a été une machine mathématique essentielle – à la fois aux États-Unis et en Angleterre, puis dans d’autres pays européens – pour la résolution numérique des équations différentielles, avant et pendant la Seconde Guerre Mondiale. Douglas R. Gregory (1813–1844), Augustus de Morgan (1806–1871), George Boole (1815–1864), et d'autres auteurs moins connus, un réseau d'algébristes renouvelle singulièrement la conception de l'algèbre, à tel point que leur travail est le plus souvent interprété comme émergence des travaux sur l'algèbre abstraite. Background: Family members are often required to act as substitute decision-makers when health care or research participation decisions must be made for an incapacitated relative. Yet most families are unable to accurately predict older adult preferences regarding future health care and willingness to engage in research studies. Discussion and documentation of preferences could improve proxies' abilities to decide for their loved ones. This trial assesses the efficacy of an advance planning intervention in improving the accuracy of substitute decision-making and increasing (...) the frequency of documented preferences for health care and research. It also investigates the financial impact on the healthcare system of improving substitute decision-making.Methods/DesignDyads (n = 240) comprising an older adult and his/her self-selected proxy are randomly allocated to the experimental or control group, after stratification for type of designated proxy and self-report of prior documentation of healthcare preferences. At baseline, clinical and research vignettes are used to elicit older adult preferences and assess the ability of their proxy to predict those preferences. Responses are elicited under four health states, ranging from the subject's current health state to severe dementia. For each state, we estimated the public costs of the healthcare services that would typically be provided to a patient under these scenarios. Experimental dyads are visited at home, twice, by a specially trained facilitator who communicates the dyad-specific results of the concordance assessment, helps older adults convey their wishes to their proxies, and offers assistance in completing a guide entitled My Preferences that we designed specifically for that purpose. In between these meetings, experimental dyads attend a group information session about My Preferences. Control dyads attend three monthly workshops aimed at promoting healthy behaviors. Concordance assessments are repeated at the end of the intervention and 6 months later to assess improvement in predictive accuracy and cost savings, if any. Copies of completed guides are made at the time of these assessments.DiscussionThis study will determine whether the tested intervention guides proxies in making decisions that concur with those of older adults, motivates the latter to record their wishes in writing, and yields savings for the healthcare system.Trial RegistrationISRCTN89993391. : Catholic teaching has no moral difficulties with research on stem cells derived from adult stem cells or fetal cord blood. The ethical problem comes with embryonic stem cells since their genesis involves the destruction of a human embryo. However, there seems to be significant promise of health benefits from such research. Although Catholic teaching does not permit any destruction of human embryos, the question remains whether researchers in a Catholic institution, or any researchers opposed to destruction of human embryos, (...) could participate in research on cultured embryonic stem cells, or whether a Catholic institution could use any therapy that ultimately results from such research. This position paper examines how such research could be conducted legitimately in a Catholic institution by using an ethical analysis involving a narrative context, the nature of the moral act, and the principle of material cooperation, along with references to significant ethical assessments. It also offers tentative guidelines that could be used by a Catholic institution in implementing such research. (shrink)

High prevalence of musicians’ physical and mental performance-related health issues (PRHI) has been demonstrated over the last 30 years. To address this, health promotion strategies have been implemented at some post-secondary music institutions around the world, yet the high prevalence of PRHI has persisted. In 2018, an international group of researchers formed the Musicians’ Health Literacy Consortium to determine how best to decrease PRHI, and to examine the relationship between PRHI and health literacy. An outcome of the Consortium was the (...) development of a new health literacy tool for musicians, the MHL-Q19, which drew from the theoretical framework of the European health literacy suite of tools, HLS-EU. The aim of the current study was to evaluate the validity and reliability of the MHL-Q19. Participants completed a battery of questionnaires, including the HLS-EU-Q16 for the assessment of general health literacy; the Musculoskeletal Pain Intensity and Interference Questionnaire for Musicians (MPIIQM); the RAND-12 quality of life questionnaire; and the General Self-Efficacy scale (GSE). We hypothesized that the MHL-Q19 would have a weak correlation with the HLS-EU-Q16; moderate correlation with the physical component scale and weak correlation with the mental component scale of the RAND-12; moderate correlation with the GSE; and finally, moderate correlation with pain interference and weak correlation with pain intensity of the MPIIQM. A total of 549 post-secondary music students from six English-speaking countries completed the battery of questionnaires, and 328 of these participants provided valid responses to the MHL-Q19 alone 2 weeks later. The tool showed acceptable internal consistency and test–retest reliability. Three of our hypotheses were supported, although the strength of the correlations varied from what we had predicted. The fourth hypothesis was not supported; our findings indicate that lower health literacy scores were weakly related to higher MPIIQM pain intensity and interference scores. The results of this study support the notion that musicians’ health literacy is a distinct construct that cannot be fully evaluated with existing health literacy tools. Given that this is a new instrument, the evidence presented is positive and promising. Further studies will be needed to refine the tool. (shrink)

Rare copy number variants have a prominent role in the aetiology of schizophrenia and other neuropsychiatric disorders. Substantial risk for schizophrenia is conferred by large CNVs at several loci, including microdeletions at 1q21.1, 3q29, 15q13.3 and 22q11.2 and microduplication at 16p11.2. However, these CNVs collectively account for a small fraction of cases, and the relevant genes and neurobiological mechanisms are not well understood. Here we performed a large two-stage genome-wide scan of rare CNVs and report the significant association of copy (...) number gains at chromosome 7q36.3 with schizophrenia. Microduplications with variable breakpoints occurred within a 362-kilobase region and were detected in 29 of 8,290 patients versus 2 of 7,431 controls in the combined sample. All duplications overlapped or were located within 89 kilobases upstream of the vasoactive intestinal peptide receptor gene VIPR2. VIPR2 transcription and cyclic-AMP signalling were significantly increased in cultured lymphocytes from patients with microduplications of 7q36.3. These findings implicate altered vasoactive intestinal peptide signalling in the pathogenesis of schizophrenia and indicate the VPAC2 receptor as a potential target for the development of new antipsychotic drugs. © 2011 Macmillan Publishers Limited. The territorial contraction and speaker-reduction undergone by the Welsh language during the past few centuries has resulted in its categorization by many linguists as an obsolescent language. This study illustrates that, although it is undeniably showing some signs of decline, Welsh stands in marked contrast to many previously documented cases of language death. Against this backdrop of contraction a steady revitalization is taking place. Based upon extensive fieldwork in two sociolinguistically contrasting communities, this book is the first to examine the (...) position and nature of contemporary Welsh with reference to both obsolescence-related developments and changes under way in the dialects. Jones focuses on immersion education, long heralded as the saviour of the language and, by examining the variety of Welsh being produced by immersion pupils, seeks to determine whether this claim is justified, or whether such pupils are in fact 'speaking immersion'. As well as discussing the recent linguistic change shown by contemporary Welsh within the language death framework, the author examines the ways in which the language has been standardized and their repercussions for language maintenance. By way of comparison these tensions and implications are also explored with reference to the other varieties of P-Celtic, namely Breton and Cornish. Marie Durand n’est pas très connue en dehors du monde protestant. Elle a passé 38 ans emprisonnée dans la Tour de Constance à Aigues-Mortes parce que son frère était un pasteur clandestin du xviiie siècle. Elle est surtout connue depuis le livre de Benoît en 1884. Mais c’est au début du xxe siècle qu’elle devient une personnification de la résistance pacifique au nom des droits de la conscience et de la tolérance et qu'elle accède à un statut d'héroïne. (...) Cela permet aussi à la Réforme un renouveau moral et spirituel. La référence à Marie Durand s'accentue en 1945 et culmine lors des cérémonies de 1968. This paper examines the accounts of limit decision advanced by Hervaeus Natalis and Durand of St. Pourçain in their respective discussions of the sanctification of the Blessed Virgin. Hervaeus and Durand argue, against Aristotle, that the temporal limits of certain changes, including Mary’s sanctification, should be assigned in discrete rather than continuous time. The paper first considers Hervaeus’ discussion of limit decision and argues that, for Hervaeus, a solution of temporal limits in terms of discrete time can coexist (...) with an Aristotelian continuous time solution because Hervaeus takes continuous and discrete time to be two non-intersecting, but correlated time-series. The paper next examines Durand’s account of limit decision and argues that Durand rejects Hervaeus’ correlation assumption as well as Aristotle’s continuous time solution. (shrink)

Depuis les années 1960, Gilbert Durand a fondé et développé une méthodologie et une épistémologie novatrices de l'étude des imaginaires individuels et culturels qui ont inspiré une Ecole de Grenoble, qui n'a cessé d'essaimer à travers un grand nombre de centres de recherches en France et dans le monde. Sa pensée, connue à travers la mythocritique et la mythanalyse, enrichie par une vaste culture historique et pluriculturelle, a été appliquée dans les domaines les plus divers des sciences humaines et (...) sociales. Loin d'être un système clos, son oeuvre a inspiré une épistémologie ouverte qui a permis, sur la base des acquis fondamentaux, de se préciser, de s'adapter à des champs nouveaux, de recevoir les apports d'autres écoles, courants ou traditions culturelles. Les contributions ici rassemblées permettront de mieux situer l'émergence de la problématique de l'imaginaire dans différentes disciplines ou champs d'études, de présenter les innovations théoriques apportées par l'anthropologie de l'imaginaire de G Durand, d'illustrer les innovations qu'elles auront permises, d'amorcer des éléments de prospective pour les chantiers actuellement en cours d'exploration.
